‼️ Reminder to parents about vaccines ‼️
ATTENTION PARENTS/GUARDIANS OF RISING 7TH GRADERS-If your student is a 6th grader moving into 7th grade the 2026/2027 school year
PLEASE NOTE:
All parents/guardians must provide a **SCDPH immunization record for their rising 7th grader showing that the TDAP vaccine (7th grade shot) has been administered PRIOR to your student starting school in August of 2026(**SC DPH Form 4024, or equal to e.g. a **notarized religious exemption or **medical exemption signed by a physician) in order for them to be able to attend school.
This immunization is made available for your student beginning on your student's 10th birthday.
South Carolina law requires that all students entering the seventh grade receive one (1) dose of Tdap vaccine. Tdap vaccine protects children from three serious diseases: tetanus, diphtheria, and pertussis (often called whooping cough). State law began the requirement for the Tdap vaccination in the 2013-14 school year.
We do not automatically receive this proof; therefore, you MUST provide proof on the appropriate forms (mentioned above) to the school. The vaccine is available by appointment at any SCDPH (also known as the health department - call (803) 385-6152) to make an appointment), the DR office (your family's Dr or child’s pediatrician) and in some cases, at Urgent Care.
Be sure to go ahead and get an appointment for this right away! Do not wait until the last minute! There are many students needing the vaccine, leading to longer waits for appointments.
Thank you for your attention to this matter!
Respectfully,
Donna Whitmire RN CMS Nurse
